*Before purchasing an item(s), please contact committee chair for approval. Reimbursement should not be assumed, please get approval first.

You cannot leave your child/ren in daycare without first completing the Daycare Emergency Form and submitting it to the office. (updated for 2011-2012)

Health Check-Ups for School Entry
Kindergartners and First Graders ONLY: This form must be completed by parents and signed by physicians prior to start of school. Please contact the office with questions or concerns. This form should be returned to the office.

Parent Handbook
The Parent Handbook is the main resource for parents and students; please read it carefully. A paper version of this document is provided to each family at our back-to-school night. If you have questions on the written material, please contact the office.

PIP Interest Form
The PIP Interest Form will let PIP coordinators know what particular volunteer events you are interested in. This is not a commitment to volunteer. This form should be returned to the office.

PIP Tracking Form
The PIP Tracking Form is where you should document all of your school and parish volunteer hours. Double and triple PIP require prior approval from the school principal. If you have questions regarding the use of this form, please contact Lisa Huch-Laughter in the school office.
