It is imperative that an emergency address and telephone number, other than the home address and telephone number, be
on file at school. This emergency information is requested the first day of school on an appropriate
card. The card should be returned immediately with all completed information.
If there is a change of address, telephone number or work place, the school office should be notified immediately.
